“Mixed” is the most common result and is an honest answer, not a guess: one Form 5500 usually bundles health, dental, life, and disability, so some benefits are insured and others self-funded. The form does not say which — read the plan's Summary Plan Description (SPD) to know how your health benefit specifically is funded.
